How can I create a "change request form"?


Currently: In adding a "case", there's an option to choose "request" or others, when a certain "request" is chosen, there's a box of description to be filled in and a file attachment is possible.


Question: Is there possibility to create a custom-made request form to apply our change management flow? Like change request form, R&D request form, or any other forms. How can I do that?

Yes, there is. You would use custom actions for this. I would recommend hopping on one of our Advanced Q&A forums (not the Intermediate ones) at and the host can walk you through how to setup a custom action form that will have the fields you would like. You can also use professional services or Quickstart hours to have a CSM do this with you one-on-one.
