Can you post a screenshot of the custom action without any confidential information?

Start Date and Due Date
I am trying to transfer the start date and due dates from a request to project. Whenever I have been creating a project in my environment the start and due dates from the request for some reason do not transfer over to the project even when we tell it to in the custom action.
