Hello Brad,
You need to pass a reference to that milestone in the $Parent field. For example "M-123456" or a field that contains a milestone reference.

Custom Action - Insert new Task into an existing Milestone
I'm trying to insert a new Task into an existing Milestone as shown below. I tried to use $Project in the Parent field, but it was not inserted into the Milestone I selected.
If I use $Name in the Parent field I get an error message saying it can't find the existing Milestone that I'm trying to insert the Task into. The Milestone name is 'Blue'

Keep in mind there is no $Milestone field, it would have to be a specific reference to a milestone as Roland mentioned.
