Automatically Add Someone who is "@ Mentioned" in a Discussion as a Follower?

Is there any way to automatically add someone who is "@ mentioned" in a Work Item Discussion as a Follower? This functionality is critical to our workflow (our existing project management software does this).

I believe this can be done by using a custom action. Did you give it a try yet? I think we did something similar a while back.
