I believe they should only be assigned as a resource in order for it to work. Do these users also have a full license?

Assigned Work Items in People module
We sometimes assign milestones to resources (without leaf tasks under them.)
Our item detail view is configured to show individuals their assigned work items in a large panel.
Right now, only tasks are showing up for those users when they see it. But as an admin, I can see the milestones. So:
- Do I have to make them a manager of the milestone or give them some other role?
- I know they can't actually work with the scheduling or edit the milestone or anything because these individuals have Team Member licenses. Does that prevent them from even seeing the milestone? They can see them in the timesheet view. So...?

No. They have Team Member licenses.

In order to see the full work plan (project and milestones) they would need full licenses. They can still report time against milestones and projects, but that view would not be accessible to them.
