If you create the report at the Work Item level instead of Project, you will be able to see Project, Milestone and Task level work items.

I am trying to to generate a report with items related to the Project level, specifically:
I also want to include in the same report items on the Task level, specifically:
One of the key triggers (filters) for the report would be a specific Task - Name.
How do I go about doing this? I have made several attempts and have thus far been unsuccessful.

Unfortunately however, unless I am doing something incorrectly, The only 'Task' level item I get to is "Task Type".
Any other thoughts / recommendations?

Unfortunately you are correct. Running a report on work items doesn't give you the hierarchy that you're looking for - it flattens the list without the structure of the Clarizen Work Plan.
We do have an Excel Plug-In available in the apps marketplace that would allow you to query your data via API and allows you to use the full flexibility of Excel. Here's a direct link to the information page and installation: http://www.clarizen.com/appsmarketplace/item/Clarizen-Excel-Add-in.html
We are also coming out with a brand new reporting module in the next few months. See attached for a high level roadmap for the Reports module.
