Feature Request: Ability to 'pre-populate' notes with templates

We would like to have the ability when someone clicks to add a Note, we can have templates for information that we would like entered. for example. When someone opens the notes they see something like the attached image.  They could then either wipe out the template to put something totally different or they can just fill in the notes where applicable within the template.
