Online acceptance reports from Clarizen

Hello,

We're using Clarizen as project management system for deploying solutions on customer sites. At the end of the project we send a report describing the work done and ask the customer for its comments with a signature: here is the acceptance report.

We'd like to digitalize this process using online forms in which customers could enter comments and send it back to Clarizen in a single clic.

Is there any solution to do so ?
