Discussions duplicating on different projects

Hello,

I have two separate projects, but for the same customer. The PM's on each are different and they are completely separate project records.

A user added a discussion to one of the projects, and somehow the entire discussion also duplicated with all it's replies on the other project.

Has anyone ever seen this? Why would this occur?
