Hi Cphalen,
The way multi-select picklists are actually stored, doesn't really enable a possibility to add a note.
Without understanding much about what you're trying to do, I will offer that you might want to consider custom objects as a more feature-rich solution here as you can add more fields, or even attach files to them.
