One of my users locked out

My user is getting the following error - This account has been locked, please wait 15 minutes

Nothing looks changed in the settings and I am loggin in fine.  Bill is paid too.

Help?

Is he using any integrations?   Outlook Add-in, SFDC integration, Clarizen Anywhere, Excel Add-in, etc?  Having an incorrect password here is likely what is locking him out of the account.  

Also, selecting to reset your password will grant immediate access instead of forcing the user to wait 15min.
